To update payment method, change, freeze ($5/month/person), or terminate your membership, please COMPLETE THE CHANGE FORM BELOW.

If you have any questions about the benefits of freezing  vs. termination, please call us.

Member Change Request Form

Families with multiples:

Filling out the form for yourself DOES NOT automatically apply it to your whole family. Make sure you click "include additional family member" for changes to apply to all accounts.

After you submit your form, check your email for a confirmation email. If you don’t receive a confirmation email, please call our office (281-580-4642) to make sure your form was properly submitted, otherwise, we will have no record of your request and will be unable to honor it.
